Please use this form to request data or reports from the WV BoS Homeless Management Information System. Your request will be triaged by executive staff who will respond to requests within seven business days with the estimated product delivery date. Please understand that, depending on the complexity of the report request, it can take up to 30+ days for delivery, so please plan your report requests accordingly.

HMIS data requests are subject to review and approval by HMIS staff and the Statewide HMIS Steering Committee (if a statewide request). Agencies that are not part of the HMIS System may be subject to additional organizational agreements and approvals.
